The Mental Health Action Program (MHAP) provides FREE client-led,
need-based case management and nurse navigation for individuals dealing with barriers to mental and emotional wellness.
Our focus is to help the Mercer County community achieve the best possible brain health.
The program began in 2018 and has worked closely with over 380 Mercer County residents to date.

What We Do
MHAP helps establish a mental health provider, primary health provider, and other health care providers as needed. We assist with navigating roadblocks to wellness and connect clients to resources including transportation, insurance, medication management, housing, food, education, and employment.
Our team also provides prevention and stress management education. This includes education about coping skills, relaxation techniques, and the warning signs of suicide and importance of early intervention.
Our goal is to help you overcome the barriers that keep you from being your best,
and connect you to the resources that will support your overall wellness.
